Caution is not a really popular skill. Here is why:
You can replace caution with a ship skill called high lookout, that has the same effect but doesn't consume vigour.
Also, even R1 caution will protect you from 100% of the ambushes (Invisible npcs that attack you).
Please note that visible npcs may still attack you, regardless of your caution rank.

Grinding How to
Ok so I assume that you want to grind caution anyway, so here's how:
You get proficiency while the skill is in use.
In another words, you can't hit the skill button, cancel and hit the skill button again. You won't gain any proficiency this way.
The good news is that you do not need to be sailing for you to gain proficiency points.
So all you have to do is sit outside any port with lots of food and activate the caution skill.
Advice 1: You need to be in one of the recommended jobs above for your grind to be faster.
Advice 2: Sit on a sea region that you charted so you get random bonus like fatigue reduction, vigour refill etc every 50 days or so.
Advice 3: Set one of your aides to the storekeeper duty and the other one to the surgeon duty. This allows you to be 100% afk.
Advice 4: Frugality skill has a lot of sinergy with this grind, so you can grind both. Also it allows you to reduce water/food consumption. Another tip is to use the paymaster, galley and food storage ship skills, if possible.
So you should check how long it takes for you to run out of vigour. That's how long you can stay afk and then return to feed your toons.
You also shouldn't overstay on the seas as the fatigue increases faster the more days at sea you are.
150 days at sea should be max but 100 days is a fair time to go back to port and restock on food and reset the days at sea number.
